How long to wait after levothyroxine before coffee?

Take it on an empty stomach and wait 30 to 60 minutes before breakfast or coffee. Keep calcium, iron, magnesium and antacids at least four hours away.

The short answer

30 to 60 minutes. Levothyroxine is absorbed best on an empty stomach, so it is usually taken first thing in the morning, with the first food or drink of the day held back for half an hour to an hour.

Why coffee gets singled out

Nothing about coffee is uniquely forbidden. The rule is the empty stomach, and coffee is simply the thing most people reach for within ten minutes of waking — before the tablet has had any time at all. Coffee with milk adds calcium to the same swallow, which is a second problem rather than the same one.

So the practical version is: the tablet, then the window, then whatever you normally have.

The four-hour rule is a different rule

Some things need much more separation than breakfast does. Keep calcium, iron, magnesium and antacids at least 4 hours away from a levothyroxine dose — they block absorption rather than merely slowing it.

This is the one people get wrong most often, because a calcium or iron supplement feels like part of the same morning routine as the thyroid tablet. If both are daily, the usual fix is to move the supplement to the evening.

Consistency beats precision

The dose you are on was arrived at by measuring your TSH some weeks after a change — usually 6 to 12 weeks after — while you were taking the tablet in whatever way you take it.

That means a routine you can actually keep is worth more than an ideal one you follow twice a week. If 60 minutes without coffee is not going to survive a Tuesday, a reliable 30 is the better answer — and if you take it at night instead, well after your last meal, that is a routine too. The failure mode is not "wrong window", it is a different window every day, because then a TSH result has nothing stable to be read against.

The trap nobody warns you about: the morning drink

The calcium tablet is easy to spot. The thing that actually catches people is breakfast in a carton — a meal-replacement, protein or fibre drink taken on the way out the door, at exactly the wrong moment.

The reason is simpler than a special interaction: it is food, and it is arriving at the exact moment the tablet needs an empty stomach. On top of that, MedlinePlus names soybeans, walnuts and dietary fibre as things that can affect how levothyroxine works for you — so a carton with added fibre or soy protein is doing more than just breaking the fast.

Don't over-apply the four-hour rule

The four-hour separation is about supplements. MedlinePlus names calcium carbonate (Tums) and ferrous sulfate (an iron supplement) — it does not say the milk in your breakfast needs four hours of clearance.

Dairy as food sits under the ordinary rule: it goes after the 30 to 60 minute window, like anything else you eat. Which still rules out the carton on the way out the door, because that gets drunk inside the window rather than after it.

What makes it hard to catch is the lag. Nothing on the carton identifies itself as an interaction, and the effect shows up a week or two later as tiredness or dizziness — by which point you are far more likely to blame your thyroid, your job or your sleep than a drink you started buying last Monday.

If your mornings have changed and you cannot think why, the question worth asking is not only "what am I taking?" but "what am I drinking, and when?"

What Thyrowise does about it

Thyrowise turns the wait into something you do not have to hold in your head. When you mark the morning dose taken, it starts a countdown — on the Lock Screen, in a widget, and on the Apple Watch — and when the window closes it tells you coffee and breakfast are fine. The wait length is yours to set; it ships at 30 minutes.

It also keeps the dose-change log, so the next lab result arrives with the context of what changed and when.

Questions people also ask

Does black coffee count?

It counts as breaking the empty stomach, which is what the window is protecting. Milk adds calcium on top of that.

What if I forget and eat straight away?

One morning is one morning; it is not a dose you have to make up or double. What is worth doing is noting it, so that if a lab result comes back odd you can see whether that week was unusual. Never change a dose yourself — that is your doctor's call.

Can I take it at bedtime instead?

Some people do, because the requirement is an empty stomach rather than a particular hour, and the evening can be easier to keep clear than the morning. Discuss the switch with your doctor first, and expect the next TSH to be the thing that confirms it worked.
